描述: While preparing for anchoring at Eastern Anchorage Singapore the OS was observed without safety goggles. During anchor lowering, the high tension on the chain and impact with the seabed can generate metal fragments/debris, posing a high risk of eye injury.
立即措施: 1. Stopped the operation briefly, instructed OS to wear goggles, and checked other crew members PPE.
2. Conducted refresher training on "Anchor Handling Safety" for all deck crew.
根本原因: 1. Inadequate Risk Assessment: The specific hazard of flying debris during anchor handling was not adequately addressed in the pre-task briefing.
2. Supervision Gap: The Officer on Watch failed to enforce PPE compliance before allowing the operation to start.
